Telehealth Consent
Last updated: October 21, 2026
This consent explains telehealth services provided by independent licensed providers through ZiraMedical, accessed using the Onvytal platform. Please read it before you start your intake. You will be asked to agree to it before your provider reviews your information.
What telehealth is
Telehealth is the delivery of health care using electronic communications. It may include an online questionnaire reviewed by a provider (asynchronous care), secure messaging, and live video or phone visits (synchronous care). In DC, KS, LA, MS, NM, RI, WV, a live (synchronous) consultation with a provider is required before treatment.
Benefits
- Access to a licensed provider without traveling to an office.
- Care at a time that works for you, with follow-up through your patient account.
Risks and limitations
- Your provider cannot do a physical exam, and information you provide may be incomplete. Your provider may decide that telehealth is not appropriate and recommend in-person care.
- Technology can fail, causing delays. Electronic systems, despite safeguards, carry a risk of unauthorized access.
- Telehealth is not for emergencies. If you think you're having a medical emergency, call 911.
What you agree to
- You will give complete and accurate information about your health, medications and allergies, and update your provider if anything changes.
- You understand that a licensed provider must review your information; if appropriate, a prescription will be issued. A provider may decline to prescribe.
- You understand that individual results vary, and that: Compounded medications are not FDA approved and are not generic equivalents of brand-name drugs.
- You will follow your provider's instructions, report side effects, and keep a relationship with a primary care provider for your other health needs.
- You confirm you are physically located in the state you listed at the time of each consultation.
- You may withdraw this consent at any time by contacting us. Withdrawing does not affect care already provided.
